Our physician and nurse practitioner led programs for Skilled Nursing Facilities, Assisted Living or other congregate housing and in individual residences. These programs can be as short as a single visit for clinical and HCC Risk Scoring assessment, short term for management of post-hospital discharged members to long term for those members not accessing the current PCP network.
