CarePlus from INSPIRIS is a multi-level approach to the coordination of care for the frail and the chronically ill residing in long-term care and assisted living facilities.

Bringing care directly to the member prevents many of the reasons for acute care hospitalizations. CarePlus provides on-site clinical expertise focused on preventative, proactive care.

Each member is assigned to an INSPIRIS clinical care team. The team is comprised of a Team Leader, a physician, a Registered Nurse Practitioner and a Clinical Care Manager. Working closely with the physician, the Registered Nurse Practitioner serves as the primary care provider, and provides “first call” coverage 24/7. Members receive frequent visits and benefit from early identification and proactive evaluation and management of acute problems.

Plan members living in a long-term care facility typically costs a plan 1.5 times the premium revenues. Implementing the INSPIRIS CarePlus program can reduce these costs by more than 50 percent, while significantly increasing quality and satisfaction outcomes.
